We are providing tools to assist youth pastors as they gather and hand out information to their groups before coming to Souled Out. Many of these tools are customizable with your own church/organization information, pricing and details. Many are in Microsoft Word 2003 format to simplify organizing your trip. Each form has an instruction sheet as the first page to explain the intent of the form and any customization issues.

The Partipant Registration Form is designed to provide a means to register students and chaperones for the conference. The information gathered is comprehensive and well laid out.

The Parent Information Form provides parents with information about:

• what this Souled Out conference is about
• the who, what, when, where and how much for this conference
• the specifics about Souled Out such as what to take and rules

The Liability Release Form provides a protection for your church/organization regarding medical, travel and photo/video issues.
